I've always wondered about that phrase.  This occurs in Revelation after the Millennial Kingdom ends.  What could it mean?  I suggest a pole shift which wipes out everything made by man and sweeps the Earth clean, if you will.  The heavens will appear (and indeed BE) new as the Earth's alignment will be new and whatever is seen above a point today will be in a very different location after the Earth realigns.... Just a thought.
