The great multitude in heaven are not just Gentiles because Rev. 7:9 says, "After this I beheld, and, lo, a great multitude, which no man could number, of all nations, and kindreds, and people, and tongues, stood before the throne, and before the Lamb, clothed with white robes, and palms in their hands; " This would include the Messianic Jews who are Christians who are living in Israel & in other nations of the world.

Galatians 3:28 also says, "There is neither Jew nor Greek, there is neither bond nor free, there

is neither male nor female: for ye are all one in Christ Jesus."  

There was a Gentile Court outside the Jewish Temple in Jerusalem which divided or separated the Gentiles from the Jews who worshipped inside the temple, & the Gentiles were forbidden to go inside the temple and worship.  Although there will be a court outside the temple of God in heaven, Messianic Jews who are Christians and Gentiles who are Christians will not be separated in heaven.  In other words the Gentile Christians will not have to remain in a Gentile court outside of the temple in heaven.

Now the entire great multitude taken to heaven will be worshipping inside God's temple in heaven according to Rev. 7:15 which says, "Therefore are they before the throne of God, and serve him day and night in his temple: and he that sitteth on the throne shall dwell among them."  This clearly shows that the great multitude of both Messianic Jews & Gentile Christians serve God day and night in his temple in heaven."

Also Rev. 3:12 says, "Him that overcometh will I make a pillar in the temple of my God, and he shall go no more out: and I will write upon him the name of my God, and the name of the city of my God, which is new Jerusalem, which cometh down out of heaven from my God: and I will write upon him my new name."

So the court without the temple in Rev. 11:1-2 is referring to the Gentile court outside the third temple which will be rebuilt in the holy city of Jerusalem that the Gentiles tread underfoot for 42 months while the Antichrist reigns on earth for 42 months in Israel when his army takes over the city of Jerusalem for 42 months & he sets up the abomination of desolation so that mankind can worship both him & his image.  In the next verse Rev. 11:3, after the angel tells John to measure the temple, but not the court outside the temple, the Two Witnesses that will prophesy in Jerusalem for 1,260 days or 42 months are then discussed.  The court given unto the Gentiles is outside the third temple in Jerusalem on Earth.

Luke 21:24 also states, "And they shall fall by the edge of the sword, and shall be led away captive into all nations: and Jerusalem shall be trodden down of the Gentiles, until the times of the Gentiles be fulfilled."

It is the holy city of Jerusalem on earth that is trodden down of the Gentiles for 42 months until the Lord returns from heaven with all His saints.  Rev. 11:2 says, "But the court which is without the temple leave out, and measure it not; for it is given unto the Gentiles: and the holy city shall they tread under foot forty and two months."  According to Strong's Greek Lexicon G3961, "To tread under foot means, trample on, i.e. to treat with insult and contempt: to desecrate the holy city by devastation and outrage Strong’s Definitions [?] (Strong’s Definitions Legend) πατέω patéō, pat-eh'-o; from a derivative probably of G3817 (meaning a "path"); to trample (literally or figuratively):—tread (down, under foot)." G3961 - pateō - Strong's Greek Lexicon (kjv) (blueletterbible.org)  The great multitude in heaven do not tread the holy city of New Jerusalem under foot for 42 months.  It is the Gentiles that tread underfoot the holy city of Jerusalem on Earth for 42 months.
