The following names are from a top orthopedic surgeon (Colin Eakin) with the Palo Alto Medical Foundation.  When I read your request, I contacted him because we live in the vicinity of Stanford Hospital.  

Not sure what an electrophysiologist is?  Possibly a neurologist (who does EMG’s)?  If so, Ealon Joelson, David Gershfield and Joe Lacy at Palo Alto Medical Foundation are good.
